Depending on the Pack Event, particular Medical forms are required for each attendee.  Please see below for the Appropriate Medical form for the event you are interested in attending. (Note: Once on file, these forms are good for one calendar year so it is not necessary to fill out a new form per event)

Youth Protection Training (Or as YPT as we refer to it) is required for any adult who will is responsible for overseeing scouts whether a registered adult leader or a parent volunteers. In the interest of the safety of our boys, we encourage all parents, and not just registered volunteers, to obtain their YPT certificate.  Training can be done online and only takes a few minutes.

This is a BSA written permission slip for use when your Den goes on a "Go See It" adventure.  Print one per scout in your den and have the parents sign on page 2 for every trip your den goes on.
